1. One duly completed by computer, signed with ink pen, and printed out (double-sided) application form. (The signature has to be original, photocopy or scancopy would not be accepted)
• For legalisation of death certificate submitted by the relative of the deceased,please fill Legalization for Death Certificate, and provide the proof of relations between the two. In the case that the application is submitted by a non-relative of the deceased, the proof of relations between the two and a letter of authorization from of the deceased's relative needs to be provided.
• For legalisation of birth certificate of a minor submitted by his/her legal guardian: please fill the applicant part with information of the minor and agent part with information of his/her legal guardian; the form need to be signed by his/her legal guardian. In the case that the application is not submitted by his/her legal guardian, please fill the applicant part with information of his/her legal guardian.
2. The original and a copy of the document to be legalized, but already legalized by the Belgian Ministry of Foreign Affairs
3. The original and a copy of the applicant's passport, Belgian nationals can also provide the original and a copy of the Belgian ID card.
4. If the application is submitted by an agent, the original and a copy of the agent's passport or ID are also required.
5. If the legalisation is regarding a company's commercial activities, please provide a copy of its legal person's passport, together with document that testify his function as the legal person in the company (e.g. document of the business registration)
6. Other documents consular officials require.
